Overview
On 10 September 2026, Yatharth Hospital, Noida Extension announced the successful completion of a simultaneous liver‑kidney transplant (SLKT) on a 50‑year‑old male patient suffering from end‑stage kidney disease and chronic liver disease secondary to hepatitis B infection.
Patient Background
The patient had previously undergone a right radical nephrectomy for a renal tumour approximately one year earlier, after which he developed chronic kidney disease and was on thrice‑weekly haemodialysis. Co‑morbidities included type‑2 diabetes mellitus with diabetic nephropathy and neuropathy, hypertension, single‑vessel coronary artery disease, and hepatitis B‑related chronic liver disease.
Donor Information and Compatibility
The liver graft was donated by the patient’s wife, who is blood group B‑positive, while the recipient is O‑positive, making the liver transplant ABO‑incompatible. The kidney graft was donated by the patient’s daughter, sharing the same blood group as the recipient.
Surgical Procedure
The combined operation lasted approximately 15–16 hours and involved coordinated efforts of liver transplant, kidney transplant, anaesthesia, blood bank and critical care teams. Both donor surgeries were performed laparoscopically. Pre‑operative CT angiography revealed unsuitable right iliac vessels in the recipient; consequently, the reconstructed kidney was anastomosed to the left iliac vessels. The kidney donor kidney possessed two renal arteries, necessitating a complex vascular reconstruction using the Pantaloons anastomosis technique. Additional challenges comprised a fatty liver in the donor and the recipient’s pre‑existing coronary artery disease.
Post‑Operative Outcome
Following transplantation, the patient exhibited good recovery, with both liver and kidney grafts functioning well. The multidisciplinary team emphasized that meticulous planning, advanced surgical expertise and seamless coordination were critical to managing the ABO‑incompatible liver graft and the patient’s multiple comorbidities.
