The Department of Gynecology and Obstetrics at Zydus Hospitals, Ahmedabad, is positioned as a leading high‑risk pregnancy and IVF centre. It operates an in‑house blood bank, a dedicated Gynec ICU, an in‑house NICU, and provides 24‑hour specialist coverage across obstetrics, fetal medicine, anaesthesia, and neonatology. The facility is designed to manage a substantial portion of the high‑risk cases arising from India’s estimated three crore pregnancies and up to 2.7 crore live births each year.

Case Study 1: Placenta Accreta Spectrum with Placenta Previa

A 30‑year‑old woman in her second pregnancy, with a prior C‑section, presented at five months gestation. Ultrasound identified a rare placenta accreta spectrum (PAS) superimposed on placenta previa. An MRI was performed to delineate the extent of placental invasion, which was found to involve the bladder wall. Delivery was executed via an upper‑segment Caesarean section, followed by an obstetric hysterectomy to control massive bleeding. A 31‑year‑old woman and her husband, after ten years of infertility, prior PMOS, irregular periods, male‑factor infertility, and high BMI, sought IVF at Zydus after three unsuccessful IUI cycles and one failed IVF elsewhere. Under the care of Dr. Raman Patel and embryologist Dr. Reitu Patel, three embryos were transferred, all of which implanted. Following early‑trimester bleeding, the couple elected to reduce the pregnancy to twins. A cervical cerclage was placed at 16 weeks, and the mother later developed gestational diabetes. At 25 weeks and 4 days—considered extremely ultra‑preterm—a Caesarean delivery was performed. The twins weighed 800 g and 900 g. Both infants required intensive neonatal support; one experienced pulmonary hypoxia and needed ventilator assistance. They remained in the in‑house NICU for approximately three months before being discharged to their mother, and are now reported to be thriving.

Additional Complex Cases

The department also managed a patient with chronic cervical insufficiency who experienced amniotic fluid loss at four months. After multidisciplinary monitoring, a laparoscopic abdominal cerclage was performed, allowing the pregnancy to continue for an additional 3.5 months. The patient delivered successfully, and both mother and child recovered well; the child recently celebrated its first birthday.
