Action Taken
The Company has completed dispatch of intimation letters to identified shareholders holding shares in physical mode, urging them to furnish:
- PAN
- Contact Details (Postal Address with PIN and Mobile Number)
- Bank Account details
- Specimen signature
Consequences of Non-Compliance
Physical security holders who have not furnished required details:
- Shall not be eligible to lodge grievance or avail any service request from the Registrar and Transfer Agent (RTA)
- Shall be eligible for any payment (dividend, interest, redemption payment) only through electronic mode with effect from April 1, 2024
- Must furnish required details before being eligible for services or electronic payments
Nomination Details
Providing 'Choice of Nomination' is optional but encouraged for ensuring smooth transmission of securities and preventing accumulation of unclaimed assets.
Required Forms for Submission
Shareholders must submit the following SEBI-prescribed forms:
1. Form ISR-1: For registering/updating PAN, Contact details, Bank Details and Signature
2. Form ISR-2: For confirmation of Signature by Banker (with original cancelled cheque or self-attested bank passbook/statement)
3. Form SH-13: For nomination details
4. Form SH-14: For cancellation/variation in nomination
5. Form ISR-3: Declaration for opting out of nomination
Submission Methods
Documents can be submitted to the RTA through:
1. Post: Hard copies of self-attested documents to Alankit Assignments Limited, Unit: Artemis Medicare Services Limited, Alankit House, 4E/2, Jhandewalan Extension, New Delhi-110055
2. In Person Verification (IPV): Authorized RTA personnel will verify original documents and retain copies with IPV stamping
3. Electronic mode with e-sign: Documents with digital signature certificate via email to rta@alankit.com
Dematerialization Recommendation
The Company strongly urges physical shareholders to dematerialize their shares in accordance with Regulation 40 of SEBI LODR Regulations, which mandates that all transfer requests (including transmission and transposition) shall be processed only in dematerialized form.
