Key Requirements for Shareholders

Physical shareholders must furnish the following mandatory details for their folio numbers:

  • PAN (Permanent Account Number)
  • Choice of nomination
  • Contact details (postal address with PIN code and mobile number)
  • Bank account details
  • Specimen signature

Forms Required for Compliance

Shareholders must submit the following forms along with supporting documents:

  • Form ISR-1: For KYC details or changes/updation thereof, including PAN registration
  • Form ISR-2: For confirmation of signature by banker
  • Form ISR-3: Declaration form for opting-out of nomination (if applicable)
  • Form SH-13: Nomination form
  • Form SH-14: Cancellation or variation of nomination (if applicable)

Forms ISR-1 and ISR-2 are mandatory. Shareholders must also submit either ISR-3 or SH-13 or SH-14 based on their nomination choice.

Consequences of Non-Compliance
