The mandate requires all physical security holders to provide:

  • PAN (mandatorily linked to Aadhaar effective July 1, 2023)
  • Address with PIN code
  • Mobile Number
  • Bank Account details
  • Specimen Signature
  • Email address registration (optional but recommended for online services)
  • Nomination details
Key Compliance Requirements:
  • Shareholders will only be eligible to lodge grievances or avail services after furnishing complete documents/details
  • From April 1, 2024, dividends/interest will be paid only through electronic mode upon furnishing all required details
  • If updated after April 1, 2024, shareholders will receive all dividends/interest declared during the period from April 1, 2024 till date of updation
Submission Methods:

1. In Person Verification (IPV): Registered shareholder(s) must visit the RTA office personally (not representatives)

2. Hard Copy: Submit self-attested photocopies of relevant documents with date

3. With E-sign:

  • Upload scanned copies with e-sign to email: kyc1@in.mpms.mufg.com (if email already registered)
  • Upload on website: https://web.in.mpms.mufg.com/KYC/index.html using Serial No. on KYC Form
Required Documentation (Form ISR-1):
  • PAN cards of all holders (self-attested with date)
  • Proof of address (if changed) through government-issued documents, utility bills (not more than 3 months old), or identity cards
  • Bank details: Original cancelled cheque leaf bearing first holder's name OR bank passbook/statement copy attested by bank
  • Demat Account Number (16 digit DPid/Client id) with Client Master List duly signed by Depository Participant
  • Specimen signature with banker's attestation (Form ISR-2) and original cancelled cheque leaf
  • Nomination forms: SH-13 (first time), SH-14 (cancellation/variation), or ISR-3 (opt-out)
