Overview

Kauvery Hospital, Tirunelveli, Tamil Nadu, announced that it successfully performed a combined beating‑heart coronary artery bypass grafting (CABG) and laparoscopic abdominal surgery in a single operative session on a 75‑year‑old male patient.

Clinical Details

The patient presented with triple‑vessel coronary artery disease and a large 10‑cm mesenteric cyst causing severe abdominal pain. The cardiac team carried out a CABG on the beating heart, creating three bypass grafts – one arterial graft harvested from inside the chest and two venous grafts from the leg – to revascularise the three blocked coronary vessels. Immediately thereafter, the gastrointestinal team performed a laparoscopic subtotal excision of the 10‑cm mesenteric cyst using keyhole incisions.

Surgical Team

The procedures were led by Dr. R. Arun Singh, Cardiothoracic Surgeon (CABG), with cardiology support from Dr. M.S.L. Mahboobu Subuhani. Dr. Karthikeyan, Gastrointestinal Surgeon, performed the laparoscopic cyst excision. Anaesthesia and peri‑operative care were provided by Dr. Lakshmanan K and Dr. Selvi Mahalingam.

Rationale and Risk Management

Because the cyst caused severe pain, the multidisciplinary team elected to address both pathologies in one session despite the high‑risk nature of immediate sequential major surgeries. Risks such as increased bleeding due to anticoagulants used in cardiac surgery, altered clotting, potential impact on cardiac function from laparoscopic insufflation, and infection were carefully assessed and mitigated. The beating‑heart technique was chosen to reduce cardiac stress, and continuous monitoring was maintained throughout.

Outcome

Post‑operative recovery was uneventful; the patient showed no signs of infection during follow‑up and continues to do well, with both cardiac and abdominal conditions resolved.

Statement from Hospital Administration

Dr. Lakshmanan, Medical Administrator, highlighted that the achievement showcases the hospital’s clinical expertise, preparedness, and teamwork, reflecting its commitment to advanced multidisciplinary care for complex, time‑sensitive cases.
